Requirements for Travel

1. PASSPORTS

  • National IDS of both parents
  • Birth Certificate of the patient
  • Passport photo of the patient
  • Passport photo of mother or father
  • 600,000/= for the passport

2. Airtickets

  • Visas for both patient and guardian
  • Medical Information Form (MEDIF)
  • Travel local
